Scholarship
| a. | Applicants must have served active or reserve duty in any branch of the military or in the National Guard. Applicants that have been discharged must hold an honorable discharge. Air Force Medics are particularly encouraged to apply. |
| b. | The applicant must be a student in good standing enrolled in a Committee on the Accreditation of Allied Health Education Programs (CAAHEP) approved program in Tennessee, or |
| c. | A student in a CAAHEP approved program outside the state of Tennessee who holds valid residency in the state of Tennessee; accepted proof will be valid driver's license from Tennessee or other authenticated legal document. |
| d. | In order to be considered, applicants must return their completed applications with requested documentation attached by the stated deadline. |
| e. | The completed application deadline is Sept. 15, 2009, for this scholarship. |
| f. | The Foundation will not be held responsible for lost or delayed applications. |
| g. | Please include with your application your most current transcript of the last semester of college attended with application. |
| h. | Verification of information may be required. False or misleading information will be grounds for application disqualification. |
| i. | Family members of selection committee members are not eligible for scholarship. |
| j. | Decision and notification of recipient(s) will occur before Oct. 20, 2009. |
| k. | Notification of recipient(s) will be done by committee via phone or mail. |
| l. | The decision of the committee is final and all applications become property of the Foundation to be held in confidentiality one year, at which time they may be destroyed. |
